2025 Fall Sports Information

All athletes must submit the following forms before beginning practice (forms available in upper right of this page):

1-Proof of physical and/or alternate year card (physical must be after April 1, 2023)
2-Permission to participate in school activities form
3-Insurance Waiver
4-Concussion acknowledgement
5-Emergency Card

Activity Fee Information

The middle school fee for activities is $30 unless a student qualifies for free or reduced lunch. If a student is on reduced lunch, the activity cost will be $12 and no charge for a student qualifying for free lunch.

Payments may be cash or by check made payable to Neenah Middle School.

Notice to Private and Home-Schooled Families 

Neenah Middle School's athletics programs are open to all 7th and 8th grade students who reside within the Neenah Joint School District boundary regardless of what school they attend or if they are home-schooled. Students who are interested in participating in football, volleyball, basketball, wrestling or track and field should follow the same registration procedures as Neenah Middle School students.
